FALL TENNIS 2021 REGISTRATION FORM

Please check all the following statements to confirm you understand and agree to all of the following conditions:
I understand that the Fall Tennis Schedule consists of 5 Sessions. I am registering for Fall Tennis Session 1 (16th August - 12th September). If I want to start at a later date, I will email Carmen Pham (firstname.lastname@example.org) for accomodations and requests. Requests are subject for review, and may not be approved. Note: If you want to register for a later session, please contact Carmen Pham via email for availability and enrollment.
Once Enrolled, you are automatically enrolled throughoutout the school year. We charge by sessions (each session is 4 weeks long, except for Session 5 which runs from December 6 - December 23). If you want to cancel enrollment you must email Carmen Pham (email@example.com) before the start of the next session. We do not accept late cancellations.
Deductions for vacation or anticipated missed classes need to be given prior to the start of the session via email in order to be adjusted from tuition. If missing a class, you will need to email Carmen Pham (firstname.lastname@example.org) at least 12 hours in advance in order to receive a make-up (we allow for 1 make-up per session). Make-ups will not be permitted for late notifications or no-shows to classes.
Any rain days are deducted from the next session's tuition. Rain days are notified through email and text (whichever contact information you provided upon registration). If you will be missing a class, we must be notified at least 12 hours in advance in order to receive a credit or make-up. If not notified, the class will be charged as usual.
I understand that only tennis participants with a tennis membership are eligible for tennis class discounts and private lesson discounts.
